For over 30 years, Anne Grogan has helped seniors and their families navigate health and social issues.
Anne Grogan earned her Master of Social Work (MSW), from Marywood University, and is also a nationally recognized certified case manager (CCM) through The Commission for Case Management Certification. She was the Director of Geriatrics for a hospital network for 30 years, designing, developing, and implementing programs to meet the needs of a growing elderly population. With her strong background in ethics, she was asked to co-chair the hospital Bioethics Committee. An industry leader, she has been asked to speak at state and national conferences. Recognizing the importance of having well-trained staff caring for our older adults, she designed educational and training programs for senior living communities, home care agencies, and community caregivers.
Her entire career has been dedicated to creating and implementing programs to benefit seniors and counseling the “sandwich generation” as they navigate the eldercare maze. In 2017, she opened her own private practice, working directly with older adults and their caregivers. Anne’s services are now available to families as a geriatric case manager, assessing the needs of the older relative and developing a game plan to help the older person “age in place.”
In addition to being a Licensed Social Worker, Anne holds certifications in elder law, bioethics and public health, and a faculty fellowship in geriatric education.

For over 30 years, Anne Grogan served the Warren County older adults and their families as the Warren Hospital Director of Geriatric Services and the St. Luke’s Network Director for Senior Care. She developed the Center for Positive Aging which included geriatric assessment, caregiver support groups and education, Future Focus Senior membership program, Express Ride Hospital Transportation, physician house calls, and the Comfort Zone Adult Daycare, the first and only adult medical daycare in Warren County. Anne also served as the Co-Chair of the hospital’s Bioethics Committee with Dr. Frank Gilly, the first non-physician appointed to that position. Together, they helped patients, families, physicians, and staff with difficult end-of-life care decisions.
Anne continues to serve Warren County through APG Eldercare Services. As a private geriatric care manager she helps older adults and caregivers navigate the often daunting eldercare maze.
Anne served on the Board of Trustees for the Phillipsburg Senior Center and was a member of the Phillipsburg Rotary Club. She was appointed a delegate to the 1995 White House Conference on Aging and the NJ Governor’s Task Force on Aging.
Anne holds a Master’s Degree in Social Work (MSW) from Marywood University and added certifications in elder law (Penn State), bioethics and public health (UMDNJ), and a faculty fellowship in geriatric education (Temple University). She is a licensed social worker and a nationally certified case manager (CCM) through the Commission for Case Manager Certification.
In the larger community, Anne sits on the Board of Lehigh Valley Aging in Place and One Call Senior Services. She is a per diem social worker with Family Pillar Hospice and Care Coordinator for Friends Life Care and Pathstones by Phoebe.
Anne is married to Patrick, who serves the Warren County community as Chief Operating Officer for NORWESCAP. They have 2 children, Patrick (Florida) and Katelyn (Nazareth). Anne and Patrick are the proud grandparents of Stella and Harper Grogan.
